To become a member of this Kairos team, please download the form, print it, fill it out and send it to the person listed on the form. Note: You can use Adobe Reader 7 or later to fill out the form on your computer, save it to disk, submit the data electronically, then print it out and complete the signature portions. You may also want to download the instructions for completing the form on your computer.

Closing Application

The closing application for Kairos 44 is available for download.
The closing ceremony is Sunday afternoon of the weekend, in the prison chapel. You must be pre-cleared to come into the unit that day. Send your completed form to the address at the bottom of the form as soon as possible. Last minute applications cannot be accepted.
Note: This PDF is a form you can fill out on your computer!
Please, if possible, do the following to insure the most accurate information is received. This will minimize the possibility of errors in the data used to clear you for coming to the closing.
- Download the electronic form.
- Open the form with Adobe Reader 7 or later.
- Complete the form as accurately as possible and double-check your information.
- Save the form to your computer's hard disk (Adobe Reader 7 or later required for this).
- Click the SUBMIT button at the bottom of the form. This will transmit the DATA on the form to our records.
- PRINT the form and sign it. Obtain your pastor's signature, if needed.
- Mail the signed form to the address at the bottom of the form.
Right-click (Mac: control-click) on the form you wish to download, then choose the option that lets you save the file to disk:
- Electronic Form: Remember, you still have to print, sign, and mail this form!
- Form for printing and hand-filling only: Use the same form as Electronic Form. When you print the form, be sure to select the option to "Print Document" (do not include "markup" or other options).

TDCJ Volunteer Application
|
|
|
To become a team member who will enter the unit you must be approved by the TDCJ. The approval process takes time. You can join the team and perform valuable duties without going into the unit, but to go in, allow at least 3 months for the approval and required 3-hour training to take place.
Note: Late in 2008, the TDCJ issued a new application form. We should be using the new form for new volunteer applications. Please be advised: The form you can download at this site is the new form! It can be filled out on your computer, then printed. This version of the "fillable" form is in test mode. If you use it, please proofread your application form carefully after you print it out. Let us know as quickly as possible if you find a problem with the fillable form. Use the "Contact Us" link to notify us of the problem.
